Becoming a Patient

1. Complete the New Patient Forms

Once your forms have been received our patient representative, Arabelle, will generate an e-chart and you will receive a time sensitive link from our electronic health record to activate your secure patient portal-- Please be sure to activate.  The patient portal is your login site directly to your e-chart and how you directly communicate with the office securely.  

3. Read our Practice Policies

4. Choose the Care Package, virtual care plan,  or fee-for-service that fits your needs best.

We believe everyone can be helped by Dr. Croft's comprehensive approach.  Getting to the root cause of a chronic condition can be expensive for some.  In effort to be accessible to all those who desire to work with us, we have partnered with Advanced Care Card.  Advanced Care offers simple and affordable patient financing solutions.  Apply today and secure the financing you need and deserve.

7. Your nominal $225 annual administrative service fee entitles you to the following benefits:

• 24-7 access via secure patient portal 

• Dr Croft's time outside of your appointment regarding your care
